Why doesn't OSO take insurance?

The short version: insurance-based clinics are paid per visit, which pushes them toward high patient volume. That's why so many patients get 15 minutes with a therapist and 45 minutes with an aide, two to three times a week. We chose a different model so we can give you one full hour, one-on-one, with a board-certified specialist every single session. Our goal is fewer, higher-quality visits to get you back to your life faster, and that's the entire point. There are great physical therapists that take insurance, we just chose not to operate that way. 

What does physical therapy actually cost without insurance?

Cash-pay rates vary widely across the Bay Area depending on session length, who you see, and whether you get true one-on-one time. Our rates are published for transparency

A 20-75 dollar copay looks cheaper per visit, but 18 visits of partial attention can cost more in total time, money, and missed progress than 6–8 focused sessions that actually resolve the problem.

Can I still use my insurance benefits?

Often, yes — just in a different way. Many patients with out-of-network (OON) benefits get reimbursed for a portion of their care. Here's how it works with us:

•     You pay for your session at the time of service.

•     We provide a superbill — an itemized receipt with the billing codes your insurer needs.

•     You submit it to your insurance company, and if you have OON benefits, they reimburse you directly. How much you get back depends on your plan's out-of-network deductible and reimbursement rate. Not sure what yours looks like?

Can I use my HSA or FSA?

Almost always, yes. Physical therapy is a qualified medical expense, so you can typically pay with your Health Savings Account (HSA) or Flexible Spending Account (FSA) card, or reimburse yourself from those accounts using your receipt. That effectively lets you pay for care with pre-tax dollars, which lowers your true out-of-pocket cost. If you have a balance sitting in an FSA at year-end, PT is a smart way to use it.

Is cash-based physical therapy worth it?

It comes down to what you're actually buying. With cash-pay specialist care you're paying for the therapist's time and expertise directly, not subsidizing a busy schedule.

For most active people, post-op patients, and athletes, that means:

•     A board-certified orthopedic specialist (only about 5–6% of PTs hold this credential) treating you the whole hour.

•     Fewer total visits, which narrows the real-world cost gap with insurance clinics.

•     No surprise bills months later, and no fighting over authorized visit counts. It isn't the right fit for everyone, and that's okay.
